Introduction

The goal of this page will be to provide templates to design and print your own Blood Component labels, including CODABAR and, potentially, ISBT-128 barcodes. This is an example of a finished label:

Required downloads

The first step you will need is to install a barcode font for your computer.
  1. Download this file FreeWareCodabarFont.zip 
  2. Extract this file using WinZip, or other software: CodabarLarge.ttf
  3. Copy  it to your Windows fonts folder. This will be either C:\WINDOWS\FONTS or C:\WINNT\FONTS
  4. Then double-click the file. If successful, you will see barcodes on the screen

Templates

After you have installed the CODABAR font open one of the templates: (You can open and view them without installing the font first, but you will see eye-readable numbers in place of the barcodes.)

These labels are designed to be printed on Avery Label #5165, which is a full sheet 8.5 x 11 label. There are light gray lines for cutting (6 labels per page, 3.5 x 4" each).

To change the barcodes, use Word's search and replace function. Replace the product code (12710) with the desired code. Replace the (1234567) with your facility ID number.

Unit number Sequence Labels (Microsoft Excel) 30/pg

These labels are designed to be printed on Avery Label #5960 or 5160 (30 labels per page). This template prints two copies of each unit number per label. The starting number can be entered. All other cells are locked for editing.
Unit number Sequence Labels (Microsoft Excel) 80/pg

These labels are designed to be printed on Avery Label #5167 (80 labels per page). This template prints two copies of each unit number per label. The starting number can be entered. All other cells are locked for editing.
Facility ID add-on labels (Microsoft Word)

These labels are designed to be printed on Avery Label #5960 or 5160 (30 labels per page). 

To change the barcodes, use Word's search and replace function. Replace the (1234567) with your facility ID number.

ABO/Rh add on- labels (Microsoft Word)

These labels are designed to be printed on Avery Label #5960 or 5160 (30 labels per page). This template prints two copies of each blood type per label. The template has eight pages, one for each blood type.
Blood Product add-on labels (Microsoft Word)

These labels are designed to be printed on Avery Label #5960 or 5160 (30 labels per page). 

To change the barcodes, use Word's search and replace function. Replace the (04750) with the desired product code.

There was an issue raised about the statement in a 1985 FDA guideline ("Uniform Labeling of Blood and Blood Components") requiring certain adhesives for blood labels. There are apparently no subsequent guidelines with such requirements, nor are they mentioned in the 21CFR600 series regulations. This is not currently a compliance issue. Click here for FDA's reply.
