FEEDBACK SHEET

SPECIAL NEEDS?

If you have special needs regarding mobility, communication, or other related health issues, please request a “Special Needs Form” from me for submission to Collette Vacations.  It must be submitted to Collette 60 days prior to departure (February 28).

Airline tickets are sold to Collette Vacations in blocks, so there is no guarantee of special seating, although you can request certain seating accommodations through Collette.  Past experience has shown that it is sometimes better to make these requests at check in.  It is your responsibility to request boarding assistance (including wheelchairs) directly with the airline.  Also, people carrying Oxygen/Sleep Apnea Machines should contact the airline directly for guidance.

__ Please send me a “Special Needs Form” to fill out.

HOMETOWN PICKUP

Because our group consists of more than ten people, we qualify for a Collette benefit called “Hometown Pickup”, whereby the company provides us with free coach transportation from a single location to Newark Liberty International Airport and returns us to that location upon return.  Our pickup location is the Vincentown Fire House, Race St., Vincentown, NJ.  You may park your car for the duration of the trip at Laufers’, a block away from the Fire House, or you may elect to be dropped off at the Fire House.  Obviously, people at distances may prefer to make their own arrangements for arrival and pickup at the airport.  More details will be provided later.  Please check your option below:

___      I will park at Laufers’ and take the coach to and from the Airport.

___      I will be dropped off at the Fire House and take the coach to and from the Airport

___      I will find my own transportation to and from the Airport

CURRENCY FOR IRELAND

The Euro is the official currency of Ireland.  I like to arrive in a country with a certain amount of local currency in my wallet.  Because we will be arriving in Ireland on a Sunday morning, it is probably a good idea to have some Euros with us (although you will find an exchange booth in the Airport – but things tend to get hectic at the airport).  While the rate isn’t the best if you buy your Euros here in the USA in advance, I feel the convenience is worth the cost.  You can make your own arrangements through your local bank or at any major airport.  I have customarily collected money in advance and then purchase Euros at an exchange center in Philadelphia for our group.  I plan to make that excursion during the week of April 10.  If you are interested in having me get you a supply of Euros, check the appropriate option below.  I must have the payments by Friday, April 8, 2005.   Make checks out to “Joseph Laufer”.  Limit: No more than $200 per person in increments of $50 only.  Suggested amount per person is $100.  Euros will be distributed at the pre-trip meeting (April 17) to those attending, or at the Airport on April 30 to those not attending.

___      I would like “first day” Euros in advance.  I have enclosed a check in the following amount:

                                    (circle one)    $ 50     $100       $150    $200    $250    $300    $350    $400

NO MATTER WHAT YOUR RESPONSES, SIGN AND RETURN THIS FEEDBACK SHEET AS SOON AS POSSIBLE
