Copy and paste the following information, and e-mail it to me.  Unfortunately, my forms don't like me, so I deleted them and went with a much simpler e-mail idea.  Make sure that everything is filled out, and use your real name.  The name you submit is the name you'll get on your certificate.  All the information MUST be accurate to facilitate communication for the duration of your course(s).  Rest assured, I do not share this information with anyone.

For in-person sign-up, you must live in or around Winnipeg, Manitoba, Canada.  While I did at one point have a teacher in southern Florida as well, she has decided, for reasons of her own, to quit teaching for now.  If the course you'd like to take is not ready at this time, I will make every effort to make it available as soon as is possible.
