Requirements for New Applicants / Renewal*

1. Three Passport-sized pictures*
2. NSO-Authenticated Birth Certificate
3. Personal Appearance (if 9-64 years of age only)
4. NSO-Authenticated Report of Marriage (for married women)
5. Other Supporting Documents (for special cases)

C. Income Tax Return (ITR)
D. School Form 137 or Transcript of Record
E. GSIS policy
F. Government Service Record
G. Seaman's Book
