Invasive Fungal Rhinosinusitis
The patient depicted here had a relapse of her Acute myelogeous leukemia.  She complained of facial pain and congestion.  Her sinus CT was not impressive, but her exam was dramatic: her right middle turbinate is seen in the middle.

The turbinate blanches as the fungal organism invades the blood vessels and kills the tissue.

A needle is seen poking the turbinate, and produces no bleeding: a sure sign of tissue necrosis.
In the same patient the middle turbinate has been resected to reveal the middle meatus lateral to it.  The uncinate and bulla ethmoidalis are blanched and necrotic as well.

The patient's middle tubinate and right lateral nasal wall were debrided.
She lived to survive this life threatening event.
