Only a qualified physician or dentist is legally authorized to prescribe medications (no matter how simple) or to diagnose conditions. No school personnel will give medications to anyone except under the following situations |
and conditions: 1. The medication has been prescibed by an authorized physician or dentist. 2. The student is under current and continuous medical supervision. 3. The medication is one that must be taken during school hours rather than before and/or after school - see note below. 4. The student is unable to remain in school unless medication is given during school hours. 5. The procedure has been followed as outlined above. |

MEDICATION ADMINISTERING PROCEDURE: School personnel must refer a request for administration of medication to the school nurse. The procedure for administering medication in the Stoneham Public Schools includes: |
1. Written orders from a physician must detail the name of the drug, dosage, and time interval medication is to be taken. 2. Written permission from the parent/guardian of the pupil must request that the school district comply with the physician's order. 3. Medication must be brought to school in a container appropriately labeled by the pharmacy or the Physician. 4. All medications are administered by the school nurse. 5. The school nurse will inform the appropriate school personnel for the potential benefits and side effects of the drug being administered. 6. A locked cabinet, desk or other similar area must be provided for the storage of the medication. |
EMERGENCY INFORMATION: Parents should ensure that the school office has current emergency information. This information should include: *Home and Work Phone Numbers for Parents/Guardians. *Other responsible adults to contact in the local area in the event that Parent/Guardian cannot be reached. *Physician's Name and Phone Number. *Medical Alert Information - All known allergies and if treatment for child is necessary. *Complete and current address. Please keep this information updated. |

REQUIRED KINDERGARTEN IMMUNIZATIONS: DPT (5) HiB (3) Polio (4) MMR (2) Screening for lead poisoning Hepatitus B (series 3) Varicella (1) or doctor's proof of chicken pox. |
Physical exam required within 6 months of entering Kindergarten. |
PLEASE NOTE: Medication that is prescribed 3x a day should be given at home in AM, after school, and at night. |

ILLNESS: Children who are ill or show signs of becoming ill should not be sent to school. A child with a fever should remain home until his/her temperature has been normal for 24 hours (without medication). If a child becomes ill at school, parents will be notified and will be expected to provide transportation for their sick child. Children will not be allowed to leave school alone and should be picked up from the school office. |

HEAD LICE: In the Fall a notice will be sent home to all parents to make them aware if we are faced with the possibility of head lice becoming a problem in our school. This is done in an effort to avoid major breakouts in our school. If head lice is discovered in your child's classroom, we will notify (those) parents with instructions. We will continue to educate your child about good hygiene. Please continue to check your child's head every night so that we can all avoid an unpleasant situation. |

FIELD TRIP MEDICATIONS: If a child MUST take a medication during school hours on days when he/she will be on a field trip, the school nurse will send home a permission slip to be signed to allow the child's teacher to administer medication under the direction of the school nurse. |
